I expressly warrant that the person named above is capable of withstanding the physical and mental demands of planned activities. I also expressly assume all risks of the activity, whether such risks are known or unknown to me at this time. I grant and convey unto Youth Songwriting Workshop and WVA all right, title, and interest in all photographic images and video or audio recordings made by WVA during my participation in Youth Songwriting Workshop.
